Provide preventive maintenance (PM) and repair services for Steris sterilization equipment at the Veterans Healthcare Administration Salt Lake City Health Care System (VHASLCHCS), located at 500 Foothill Drive, Salt Lake City, UT 84148. Only new OEM medical supplies, equipment, and replacement parts are acceptable; no remanufactured or gray market items will be accepted. The offeror must be an OEM or authorized dealer and have Steris equipment certification, verified by an authorization letter or supporting documentation from the OEM. All maintenance, parts, software licensing, warranty, and services must comply with the OEM's terms and conditions. Quotes must include a completed pricing schedule and a Steris authorization letter. Only emailed quotes will be accepted. This requirement is set aside for Small Businesses under NAICS 811210. Evaluation will be based on price and provision of the Steris Authorization Letter.
