Repair approximately 300 linear feet of concrete cracking at the 'Alert Pad' apron pavement for the 117th Air Refueling Wing, Birmingham, Alabama, and remove and replace 5,000 linear feet of existing apron expansion joint. The contractor shall provide all personnel, equipment, supplies, supervision, and any other items and services necessary to perform the work as detailed in the Statement of Work. Offers must include a signed and completed SF 1442 (including Offer Section Blocks 14-20), a completed Section B CLIN Schedule with pricing, completed representations and certifications or confirmation of current registration in SAM.gov, a minimum of two and a maximum of three past performance references for recent and relevant projects, and acknowledgment of all amendments issued. Offerors are responsible for monitoring for the release of any amendments and must be registered in the System for Award Management (SAM.gov) to be eligible for award.
