The contractor shall provide all labor, equipment, material, transportation, supervision, services, and items necessary for the removal and disposal of approximately 1,500 square feet of existing flooring (approximately 60% carpet and 40% tile) and the installation of new tile flooring at Coast Guard Station Quillayute River. Bidders must submit a complete quote consisting of: (1) a completed and signed Standard Form (SF) 1449 with the total firm-fixed-price bid; (2) a detailed price breakdown (not exceeding one page) separating costs for removal of existing flooring, preparation of surface for installation of new flooring, installation of new flooring, site cleanup, and labor hours & wages; and (3) a technical proposal (not exceeding two pages) demonstrating understanding and ability to meet the requirements, including a step-by-step technical approach, a list of major materials and equipment (including technician and firm certification, test instruments list and calibration certificates), and a high-level project schedule showing major milestones from the Notice to Award through completion within the required period of performance. Quotes must not include extraneous materials and must be submitted electronically. The project is set aside 100% for small business under NAICS code 238330 - Flooring Contractors.
