The near-site clinic MUST be located within the city limits of Georgetown, Kentucky. 2. The clinic MUST be able to accommodate full-time office hours for the City's employee health plan group which consists of approximately six hundred (600) participants including employees and their dependents. Full-time office hours mean the clinic shall be open for eight (8) to nine (9) hours per day, Monday through Friday. 3. The clinic MUST be staffed by licensed medical doctors. Nurse Practitioners and Physicians' Assistants who work in conjunction with and are supervised by licensed medical doctors are acceptable as supplementary staff. 4. The clinic and its medical providers MUST provide free covered medical services, free covered labs, and free covered generic medications to the City's employee health plan members according to the agreed-upon member services fees with the City. A list of required medical services and labs is included in the attached appendices (Appendix 1 - Services, Appendix 2 - Labs, Appendix 3- Medications). 5. The clinic MUST include an on-site dispensary so the City's employee health plan members may receive their free medication while present for their appointment. A list of medications required to be stocked in the on-site dispensary is attached in Appendix 3. 6. Telehealth services must be provided as an option to members. 7. The clinic MUST have the ability to perform physicals to satisfy the requirements of a Commercial Drivers' License and the Department of Transportation. 8. Clinic staff shall be made available once annually to perform on -site (on the City's premises) blood draws for health-risk assessments and annual flu vaccines, at no additional cost to the City. 9. Membership fees and the cost of medical services, labs, and generic medications SHALL be included in a FIXED monthly fee per member. The City shall not pay any extra or hidden charges. All fees MUST be included in the price proposal provided by the respondent. 10.
