CR#:2137260 NYS OPWDD- Long Island DDSO intends to procure a roof replacement (per the attached scope of work) pursuant to its discretionary purchasing authority. When submitting your quote, include the attached quote proposal form and a detailed cost breakdown showing the materials (detailed breakdown of materials required) and/or equipment costs and labor costs in accordance with the project specifications and prevailing wages. A cost breakdown form is provided for you (see documents tab). This is a NYS Public Works Project and Prevailing Wages MUST be paid. Vendors / sub-contractors must be registered with NYS DOL and a copy of the certificate must be sent with your quote (see guidelines in the documents tab) Any questions or requests for clarification regarding the information provided may be directed to Brooklyn.CMM.Bids@opwdd.ny.gov DESCRIPTION: Facility Address: 2 Marshmallow Drive Commack, NY 11725 Contact Phone Number: 718-264-3915 / 3918 Summary of Work: Replace roofing , fascia, soffits, gutters. (Remove and replace roofing and gutters for the house.). The vendor shall provide all labor and materials necessary to complete the work in a professional manner according to scope of work and architectural drawings if supplied. Work is to be completed within 14 days of award, unless modified below. This is a NYS Public Works project and prevailing wages are to be paid . Refer to attached specification for the following scope of work. Site must be cleaned and all materials secured at the end of each work day. Absolutely no scraps, especially fasteners, are to be left unsecured . Upon completion of (Remove and replace roofing, replace soffits, gutters, and leaders), before removal of equipment and scaffolding, contractor is to schedule a quality control and quality assurance inspection with Plant Superintendent. SCOPE OF WORK includes, but is not necessarily limited to the following: 1. Contractor is responsible to verify all measurements and quantities. 2. Construction drawing, sketches, and or plans maybe supplied and are to be followed along with the scope. 3. Remove and dispose of all roofing, and all related materials and accessories. Contractors at site visit are responsible for taking measurements of roofing square footage and supply their own ladders and equipment needed to perform take off for measurements of square footage. Contractors must were proper OSHA required PPE while on roof at all times. 4. Replace any rotted roof deck sheathing with new 5/8" thick exterior grade plywood sheathing (approx. 10 sheets). 5. Furnish and install ice and water adhesive membrane shield (or equal) over entire roof deck sheathing. 6. Furnish and install new drip edge. 7. Furnish and install Timberline UHDZ asphalt shingles (or equivalent) and new ridge venting. 8. Install new fascia and soffits, repair supports were needed. Furnish and install new vented soffit and cap all fascia with white wood grain textured aluminum. 9. Repair any rotted wood and cap all trim around doors and windows. 10. All roofing materials, accessories, installation and fastening shall be installed following manufacturers' requirements. 11. Furnish and install new aluminum flashing and caulking around chimneys, all vent pipes, all penetrations or where required for prevention of water infiltration . 12. Furnish and install new five (5) inch aluminum seamless roof gutters, leaders and downspouts. Furnish and install new Amerimax metal mesh leaf gutter guards (or equivalent), gutters shall be installed properly pitched to new downspouts. 13. Contractor will supply any third -party inspections and certifications if required. 14. All work to be done, to NYS building codes and regulations. 15. OPWDD is the AHJ for the site. Prior approval is required for all substitutions. 16. Contractor must have a Supervisor & Competent worker present while project work is being performed.
