Site work and renovation of an elderly care / assisted living facility in Anderson, Indiana. Completed plans call for the renovation of a elderly care / assisted living facility; and for site work for a elderly care / assisted living facility.

Interior and exterior building renovations. Willow Lane Hall - Handrails and Wall Protection Remove and dispose of existing handrails, existing wall protection, wall base and wallpaper if applicable. Will save handrail backer boards. Prep and paint these backer boards one coat primer, two finish coats (SW 7008 Alabaster - semi-gloss finish). Prep wall for painting level 4 finish . One primer and two finish coats (SW 7050 Useful Gray - Satin Finish). Prep and paint hall side of doors, jambs and trim . (2) coats (SW 7008 Alabaster - semi-gloss finish). Prep walls for installation of new wall protection, Handrails and corner guards (CS Acrovyn SCR-48N , color: Putty 1472) Provide and install new millwork wall base (Johnsonite Inflection 5 1/4 " MW-XX-G vinyl cove base - Mist 27). Willow Lane Hall - Resident Room Renovations (16) rooms 301-317 Remove and dispose of existing wood chair railing. Patch walls. Where chair rail is removed around the wall protection at bed head rails. Install an InPro wall protection cap at the top of wall protection (Color TBD). Prep wall for painting level 4 finish . One primer and two finish coats (SW 7006 Grey Matters - Satin Finish). Prep and paint inside of hall doors, jambs, trim and bathroom doors both sides, jambs and trim. (2) coats (SW 7008 Alabaster - semi-gloss finish). o Prep and paint ceilings (2) coats of (flat white). Remove and dispose of existing wardrobes and nightstands. Provide and install (16) new wardrobes and (29) new nightstands. (Nightstands - Maxwell Thomas-Scottsdale Cabinet, 3 drawers-Casegood Finish: Cherry Blossom- Pull color Nickel, no locks). (Wardrobes Maxwell Thomas Scottsdale Wardrobe, 2 drawer - Casegood Finish - Color Cherry Blossom - Pull color Nickel - (1) door). Will buy these from Direct Supply, contact: Angela Zych 877-797- 0852. (delivery time is normally 28 days). Remove and dispose of (12) existing bathroom tubs. Cap off all plumbing. Repair walls and paint, Remove and dispose of all flooring in these (12) bathrooms. o Provide and install (30) ADA white toilets with split seats. Provide and install new (30) ADA wall mount sink vanities with skirts and angled wall braces - White. Cultured marble top with integral sinks - White. (Faucet - MOEN #8938 - Polished Chrome). Sink drain needs to be a grated strainer only, nonoperable. Provide and install new water lines, drains and 1/4 turn shut off valves at all fixtures. Provide and install ADA pipe wraps. o Provide and install new in-wall toilet paper holders (move to door wall). Patch prep and paint where old paper hangers were installed. o Provide and install new LVT flooring and base (Shaw Abide 4107V -Conch Shell 07140) (Johnsonite 4" traditional vinyl cove base - Shoreline). Remove and dispose of existing lights in restrooms. Provide and install (30) (Miseno 12" LED Flush mount light FLSAT12LEDCCTBN). o Prep and paint all walls and ceilings. (SW Useful Gray 7050 satin finish) (Ceilings Flat White). Doors and Trim (SW Alabaster 7008 - gloss finish) o Provide and install (30) new (stainless steel trimmed mirrors. 24" x 36"). Provide and install new exhaust fans (white-Contractor's choice - new vents must cover existing vent opening). Bonds: 10% Bid Bond, Certified Check required, 100% Performance Bond, 100% Payment Bond. Contactor will be responsible for providing test painting of all surfaces in all areas. Contractor will need to get written approval for facility Executive Director before proceeding. Contractor will be responsible for removing and replacing any furniture or fixtures as needed to facilitate installation of any of the above listed work scopes. Contractor will be responsible for providing waste disposal containers for construction debris. Contractor will be responsible for cleaning all construction areas daily and complete a final clean in all completed work areas. Contractor will provide a detailed Operations and Maintenance Manual (in a 3-ring binder - 2 copies) to the Project Manager at project completion. Final retainage will be held until this O& M manual has been delivered to Project Manager. Contractor will be responsible for obtaining any permits that may be required per local building codes.
