Reference Number 0000433321 Resurfacing of three tennis courts for the City of Lebanon Recreation Department. The City of Lebanon, TN invites bids for the resurfacing of three tennis courts on a non-exclusive basis, as set forth in this Invitation to Bid. The successful bidder will be responsible for the following major elements: 1. Thoroughly clean by pressure washing all courts 2. Remove nets and posts from all three courts 3. Repair structural cracks: a. Fill cracks with Portland Cement and Acrylic Crack Repair. b. Go back on the top of cracks with Faultline Crack Repair Tape System 4. Jackhammer out all existing 6 net posts 5. Install 3" schedule 40 sleeves in concrete net footer for new net installation 6. Install 2 layers of Black Acrylic AR Base 7. Install 2 layers of color acrylic for 3 tennis courts. Design to mimic US Open green and competition blue 8. Install 2" white acrylic tennis lines 9. Install new nets and net posts 10. Vendor must remove all debris from removal and construction (inside and around the perimeter of the fenced area) of the tennis courts. 11. Warranty for workmanship and material must be stated. 12. An onsite visit is recommended before bidding on this project. (see contact information listed above) 13. Vendor must have a contractor's license. (include a copy in bid packet) 14. Vendor must include a copy of worker's compensation and liability insurance (include a copy in bid packet). 15. Vendor must include three references for projects completed within the last two years (include in bid packet). 16. After completion of work, the vendor and city representative must complete a walk through check list to approve the finished project before invoices are paid. 17.
