All bids must be accompanied by an Affidavit of Non-Collusion and a Business Relationship Affidavit. *Bid Date Desc: All bids shall be plainly marked "SEALED BID" with the bid number, closing date and time written on the outside of the sealed envelope. (*ONLY ONE BID PER ENVELOPE*) All late bids will be returned unopened. There will be No Faxed or E-Mail Bids Accepted. Bids must be marked sealed bid with bid number & submitted in person or by mail only The Board of County Commissioners reserve the right to reject any and all bids. Furnish and installation of a complete onsite wastewater treatment and disposal system in accordance with all applicable requirements of the Oklahoma Department of Environmental Quality (ODEQ), Chapter 252:641, and all local regulatory requirements. Vendor/bidder must be Oklahoma licensed on-site system installer Due to site limitations, soil conditions, and available installation area, the proposed disposal field must fit within a designated footprint measuring approximately 12 feet by 100 feet ( 1,200 square feet maximum ). The successful contractor shall verify all dimensions, elevations, utility conflicts, and site conditions prior to installation.
