The work consists of furnishing and installing all materials, labor, and equipment necessary to resurface or rehabilitate streets or roads as listed. This includes utility cover adjustments, water gate box lowering and raising, tack coat, shim, and overlay, driveway preparation and lawn and driveway restoration, pavement removal, common excavation, aggregate base gravel, erosion and traffic control, and cleanup. All work, including final loaming, must be completed by a designated date. The contractor must provide all materials and labor unless otherwise specified. Bidders must register with the Purchasing Agent and attend a mandatory pre-bid conference. The proposal must be submitted in a sealed envelope marked appropriately and include a bid security. A Performance Bond and a Labor and Material Payment Bond will be required. The contractor must give personal attention to the work, keep it under personal control, and not assign or sublet the work without written consent. The contractor must also ensure inclusion of qualified minority businesses, womens businesses, and labor surplus area firms in any subcontracting. Proposals must be signed and state the bidder's name and address, as well as all principals involved. The City reserves the right to negotiate with the low bidder, waive formalities, accept or reject any proposals if in the public interest, and set off unpaid amounts owed by the contractor against the contract price. All proposals must cover all items of work as called for, with prices including all necessary labor, tools, and equipment.
