Renovation of a recreation facility in Washington, Missouri. Conceptual plans call for the renovation of a 5,000-square-foot recreation facility.
Project Number - 21-8769/COC-MO-12-21 The Scope of Work includes an approximately 5,000 SF roof structure over the existing main stage at the Washington Fairgrounds and other incidental items and work described in the Project Manual, or reasonably inferable therefrom. The City of Washington anticipates issuing the notice to proceed in December 2021. Each bid must be submitted on the prescribed form and also accompanied by the following: - Bid Form Proposal - 5% Bid Bond or Certified Bank Check - Subcontractor Approval Form - Worker Eligibility Verification Affidavit (page NTB-1, NTB-2 & NTB-3) Contact Joe McGowan at Cochran for any questions/clarifications on the project. 636-584-0540 jmcgowan@cochraneng.com Addendums will be provided via email addresses. - Project Completion: - June 15, 2022 - Liquidated damages of $500 per day past June 15th, $2,000 per day after July 15th - 100% Performance-Payment Bond - 100% Material Bond - Insurance - Worker's Compensation for all his/her employees - Public Liability and Property Damage in the amounts listed on the Bid Documents - Certificate of Insurance; Required to be on file prior to commencing with any work - Sales Tax: The City of Washington will provide a State of Missouri Project Tax Exemption Certificate - Prevailing Wage: Contractors have to pay the State wage rate, per job classification. Contractors will have to turn in weekly certified payroll reports. - Application for Payment; Applications should be submitted on AIA Form or equivalent, a 5% retainage will be held back until project completion. Certified payroll records from the general and all subcontractors are required along with lien waivers, shall be submitted with payment applications. An affidavit for compliance with prevailing wages must be submitted with the final applications. - The successful bidder must submit a sworn affidavit and documentation affirming the business entity's enrollment and participation in the federal work authorization program and that all of its employees working for the contracted services are not illegal immigrants. - Upon signing the contract, the Contractor and any subcontractor performing the work shall provide a 10-hour Occupational Safety and Health Administration (OSHA) construction safety program for their onsite employees, which includes a course in construction safety and health approved by OSHA or a similar program approved by the Missouri Department of Labor and Industrial Relations which is at least as stringent as an approved OSHA program. All employees are required to complete the program within 60 days of beginning work on such construction project. - Missouri Law requires all bidders under a construction contract of any public works project to employ workers from Missouri or from other nonrestrictive states. Bidders may review this requirement by checking the website of the Missouri Department of Labor and Industrial Relations. - This meeting agenda will be provided as an attachment to an Addendum.
