Site work and paving for a road / highway in Granbury, Texas. Completed plans call for site work for a road / highway; and for paving for a road / highway.
Removal of existing pavement and unclassified excavation (various depths), preparation of subgrade, installation of flexible base (various depths, approx. 19,665 SY), and installation of asphalt pavement (approx. 2,240 tons) for reconstruction of existing streets with the City of Granbury, and various other items of construction associated with street construction. The work covered by these contract documents specifications and details consists of furnishing all labor, equipment, materials, traffic control, and safety devices required for street construction. The work shall include the excavation and replacement of the existing pavement and subgrade with a new asphalt pavement section as indicated on the drawings as well as all other miscellaneous items of construction, to be performed as outlined in these documents and the standard specifications, which are necessary to satisfactorily complete the work. Each Proposal shall be accompanied by a Cashier's Check or Bid Bond, acceptable to the Owner, in an amount not less than five percent (5%) of the total amount bid, as a guarantee that the successful bidder will enter into a proper Contract and execute Bonds on the forms provided in the Contract Documents. Bid sureties will be returned to all but the three lowest acceptable bidders within five days after the opening of Bids. Remaining bid sureties will be returned after complete execution of the Contract. The surety company providing the Bid Bond must conform to the same requirements for surety companies providing Performance and Maintenance Bonds, and Payment Bonds described below. The successful proposer must furnish Performance and Maintenance Bond, and Payment Bond on the forms furnished with the Contract Documents in the amount of one hundred percent (100%) of the total Contract price, from a surety company holding a permit from the State of Texas to act as surety. The surety company must have a minimum Best's Key Rating of "B" or better. The surety company, the agency and agent issuing the Performance and Maintenance Bond and Payment Bond must be authorized to issue such bonds in Texas in an amount equal to or greater than the total contract price. Owner reserves the right to reject any or all proposals, including without limitation, nonconforming, nonresponsive, unbalances, or conditional proposals. Owner will reject the proposal of any proposer that owner finds, after reasonable inquiry and evaluation, to not be responsible. If proposer purports to add terms or conditions to its proposal, takes exception to any provision of the contract documents, or attempts to alter the contents of the contract documents for purposes of the proposal, then the Owner will reject the proposal as nonresponsive; provided that Owner also reserves the right to waive all minor informalities not involving price, time, or changes in the work. The Owner award the contract for the work, such award shall be to the responsible proposer submitting the highest ranking proposal. Evaluating proposals, Owner will consider whether or not the proposals comply with the prescribed requirements, and such alternates, unit prices, and other data, as may be requested in the proposal price form or prior to the Notice of Award. The selection criteria and the evaluation summary shall total 100 points and consist of the following categories. Some of the requirements listed below expand information requested in these documents. The Owner and Engineer reserve the right to verify all information by contacting any current or past projects listed. Proposal price (50 points) - see enclosed Proposal Price Form. The proposer's past relationship with Owner (25 points). Current and completed projects and references (20 points) - see enclosed Experience Record Form. Financial capability to perform work (5 points -pass/fail). Owner may conduct such investigations as Owner deems necessary to establish the responsibility, qualifications, and financial ability of proposers. Provision of Texas Ethics Commission Form 1295 ("TEC Form 1295") by Bidders: Effective January 1, 2016, pursuant to Texas Government Code 2252.908 (the "Interested Party Disclosure Act" or the "Act"), the Owner may not award the contract to a bidder unless the bidder has provided to the Owner a completed, signed and notarized TEC Form 1295 which has been assigned a certificate number by the Texas Ethics Commission (the "TEC"). Pursuant to the rules prescribed by the TEC, the TEC Form 1295 must be completed online through the TEC's website, assigned a certificate number, printed, signed and notarized, and provided to the Owner. The TEC Form 1295 may accompany the bid or may be submitted separately, but must be provided to the Owner prior to the award of the contract. For purposes of completing the TEC Form 1295, the entity's name is City of Granbury, the contract ID number is 6523-12; and the description of goods and services is 2019 Street Improvements Project. Neither the Owner nor its consultants have the ability to verify the information included in a TEC Form 1295, and neither have an obligation nor undertake responsibility for advising any bidder with respect to the proper completion of the TEC Form 1295.
