This project includes the removal and replacement of concrete sidewalks damaged by tree roots throughout the Municipality. Proposals shall be delivered in a sealed envelope and clearly marked on the outside with the words "Root Damaged Sidewalk Replacement Project-B". Prevailing wages will not be required to be paid on the root damaged sidewalk maintenance contract. This project involves the excavation and installation of a vegetated swale and rip-rap channel with geotextile fabric; the installation of storm structures and storm pipe; grading; site restoration; and general erosion and sedimentation controls, complete in place. Proposals shall be delivered in a sealed envelope that is clearly marked on the outside with the appropriate wording GOLF COURSE SWALE PROJECT. Pennsylvania Prevailing wage rates will be required on the golf course swale Project Contract. Performance, Maintenance, and Labor and Material Payment Bonds, along with Public Liability and Property Damage Certificates of Insurance in the amounts specified, as well as Certificates of Workman's Compensation must be filed with the executed Agreement upon acceptance of the Proposal from the successful Bidder. Proposals to receive consideration must be accompanied by a Certified Check or Bidder's Bond from a Surety Company authorized to do business in Pennsylvania, made to the order of the Municipality of Mt. Lebanon in an amount equal to ten percent (10%) of the total amount of the Proposal as a guarantee that, if the Proposal is accepted, the successful Bidder will enter into an Agreement within 15 days after Notice of the Award of the Contract. The Proposals must be made to the Municipality of Mt. Lebanon, Allegheny County, Pennsylvania, and shall remain firm for a period of sixty (60) days. No Bidder may withdraw his Proposal during the sixty (60) day period without forfeiting his Bid guarantee. The Owner reserves the right to reject any or all Proposals, or any part thereof, for any reason, and also reserves the right to waive any informality therein.
