Project includes, but not limited to: Furnish all labor, equipment, materials, and incidentals necessary to provide Dam Reconstruction located at Bastrop State Park in accordance with the Request for Competitive Sealed Proposals and Contract Documents. Proposals will be evaluated for "best value" as determined by the Owner based on established criteria identified in Section 3 of the Competitive Sealed Proposal. The Project construction estimate is $5.9 million - 6.5 million. All work shall be completed within seven hundred thirty (730) calendar days commencing on the date specified in the Notice to Proceed.

An authorized agent of the business entity must sign the printed copy of the form and have the form notarized. The completed Form 1295 with the certification of filing must be submitted to the Owner within ten (10) calendar days following Notice of Intent to Award.
