Control Number: 2058111 MWVCAA, in keeping with its mission to provide efficient and effective services, is now soliciting bids from qualified, licensed and insured entities to provide abatement of asbestos services. The removal and disposal services shall be performed in the basement of the Agency's main office building located at 2475 Center Street in Salem, Oregon. MWVCAA is a federal and state funded non-profit. This project is subject to the federal Davis Bacon Act rules and regulations. For more details, see the Davis Bacon Act document in the Attachments Section. These Davis Bacon Act clauses should be considered as part of any and all agreements or contracts regarding this project, and will be included in the final written agreement or contract, signed by both MWVCAA and the contractor awarded the bid. MWVCAA is subject to the Davis Bacon Act and follows its rules and regulations when applicable. All bids exceeding $2,000, that include labor hours, will require a signed statement by the bidder (by signing our proposal) that all provisions of the Davis Bacon Act (40 U.S.C. 276a) shall be upheld and complied with. If the vendor is not familiar with the Act and its specifics, MWVCAA can provide further information or resources to the bidding vendors. It is the vendor's responsibility to fully understand what is expected to fulfill the Davis Bacon Act requirements for the bid to be accepted. Please see copy of the Davis Bacon Act excerpts in the (Attachments) section. Please submit the following (4) documents with proposals. Proposals must include: (1.) a completed Proposal Cover Sheet located at the end of the RFP instructions document, (2.) a completed and signed Davis Bacon Checklist for Vendor/Contractor in the (Attachments) section, (3.) a completed Contractor Qualifications Checklist, and (4.) a completed IRS W-9 form. bid should include a detail of the workers' position titles and the Federal Davis Bacon prevailing wages assigned to each position. A planning meeting will be scheduled with the awarded bidder for a mutual agreement of the Davis Bacon prevailing wage determinations and a preliminary timeline schedule. The awarded bidder will be considered the Main Project Manager, and as such, will assume responsibility over all sub-contractor agreements and their compliance with the Davis Bacon Act. MWVCAA's main contact will be Helena Haytas, Chief Operations Officer. The Timelines attachment includes contact number and email address. MWVCAA shall retain free access to the project site at all reasonable times, during construction and inspection with due consideration of the work in progress. MWVCAA intends to award a contract to the business deemed most qualified and advantageous to MWVCAA. Submitted bids should include licenses, permits, certifications and any other documentation required by the State of Oregon to conduct business as a qualified contractor in the business of the solicited project. We encourage all QRFs, small & emerging businesses, and minority-owned or woman-owned businesses to participate.
