Any bid received after the presiding official declares the time for submitting bids closed will not be considered and will be returned unopened. Contractor must submit prices for ALL Divisions and Alternates (if any) for a bid to be considered complete. The project scope consists of asphalt milling, HMA resurface and full depth replacement, sidewalk, ADA ramps, curb, pavement markings, landscaping, and other appurtenant work for the following sections of streets: 1. Division 1 - Spring-land Avenue from IN-212 to Royal Road. 2. Division 2 - Tall Timbers Drive from Spring-land Avenue to South Dead End. 3. Division 3 - Tall Timbers Court from Tall Timbers Drive to West Dead End. 4. Division 4 - Willow Springs Drive from Springland Avenue to South Dead End. 5. Division 5 - Willow Springs Court from Willow Springs Drive to West Dead End. 6. Division 6 - White Birch Drive from Tall Timbers Drive to Willow Springs Drive. 7. Division 7 - Barker Avenue from West Dead End to Woodland Avenue. 8. Division 8 - Vail Street from Michigan Boulevard to Spring-land Avenue. The award of the Contract will be made on the Base Bid of ALL Divisions and any selected Alternate Bids. Bids shall be properly and completely executed on Bid Forms contained in the Contract Documents. Each Bid shall be accompanied by a completely filled out Form No. 96 (Revised 2013), and acceptable bid security. Any Bid not accompanied by the aforementioned required items shall be deemed to be a non-responsive Bid. No Bidder may with-draw the proposal with-in a period of 90 days following the date set for the receiving of bids. The City reserves the right to retain any and all bids for a period of not more than 90 days, and said bid shall re-main in full force and effect during said time. The City further reserves the right to waive informalities and to award the Contract to any Bidder, all to the advantage of the City of Michigan City as determined by the Board of Public Works and Safety, or to reject all bids. Each bid shall be accompanied by an acceptable certified check made payable to the City of Michigan City, in care of the Office of the Controller, or an acceptable bid bond in the amount equal to five percent (5%) of the total bid price executed by an incorporated surety company in good standing and qualified to do business in the State of Indiana and whose name appears on the current Treasury Department Circular 570. A Performance Bond and Payment Bond each in the amount of 100 percent of the Contract price will be required from the Contractor to whom the work is awarded. Standard Questionnaire Form 96 - Revised 2013, completely filled out and signed, including attachment of Contractor's Financial Statement. In accordance with Sec. 66-130; 66-140--66- 147; and 66-103 of the Michigan City Municipal Code, each bidder will be required to com-ply with the Michigan City Equal Opportunity Program in order for the City of Michigan City to consider its bid. The successful bidder will be required to com-ply with Sec. 66-130; 66-140--66-147; and 66-103 of the Michigan City Municipal Code.
