First-Tier Subcontractor Disclosure forms will be received at the above-mentioned location and date, until 4:00 PM, local time. The work contemplated consists of furnishing all labor, materials and equipment necessary for the construction of approximately 1,300 linear feet of 12-inch diameter ductile iron pipe located within City of Beaverton public right-of-way on SW Vermont Street from SW Nicol Road to a cul-de-sac entrance for Oregon Episcopal School. The work includes installation of a pipe bridge cased crossing (helical piles, concrete pile caps, fabricated structural steel supports, steel casing and insulators, and carrier pipe insulation), furnishing and installing restrained and push-on ductile iron pipe, fittings, valves, fire hydrant assemblies, water service connections including a 6-inch service meter vault, and connections to existing waterlines, including abandonment of existing waterlines. No proposal will be considered by the Raleigh Water District unless the Bid contains a statement by the Bidder that the Bidder will comply with the provisions of ORS 279C.800 to 279C.870 and 40 U.S.C. 276(a) pertaining to prevailing wages, as applicable. Contractor licensing under ORS 468A.720 for working with asbestos-containing materials is not a requirement of this project. Each bid must contain a statement as to whether or not the bidder is a resident bidder as defined in ORS 279A.120. Each bid must also contain a statement as to whether the bidder is registered with the Oregon Construction Contractors Board. No bid will be received or considered unless the bidder is registered with the Construction Contractors Board, as required by ORS 701.035 et seq. No bid will be considered unless the First-Tier Subcontractor Disclosure form is submitted by the specified deadline. Faxed copies will NOT be accepted. Contractors must be qualified in accordance with the applicable parts of ORS 279 in order to enter into a contract with the Raleigh Water District for public work in Oregon. To document qualifications, bidders shall submit a completely executed Contractor's Prequalification Application when submitting their bid proposal. Prequalification application forms may be obtained at the State of Oregon Department of Administrative Services, Purchasing Section, 1225 Ferry Street SE, Salem, Oregon 97301-4285 or on-line Proposals must be submitted on the prescribed forms and must be accompanied by certified check, cashier's check, or bid bond executed in favor of Raleigh Water District in an amount not less than five percent (5%) of the amount bid. The successful bidder will be required to furnish to the Raleigh Water District a performance bond and a payment bond each equal to one hundred percent (100%) of the amount of the bid. The successful bidder and all subcontractors will be required to furnish to the Oregon Construction Contractors Board a public works bond in the amount of $30,000, unless exempt under ORS 279C. Raleigh Water District reserves the right to reject any and all bids, to waive any irregularities, and to accept the bid deemed in the best interests of Raleigh Water District. Raleigh Water District may reject any bid not in compliance with all prescribed public bidding procedures and requirements, and may reject for good cause any or all bids upon a finding of the agency it is in the public interest to do so.
