The City of Ontario's Municipal Utilities Company (OMUC) proposes a pressure reducingstation in recycled water zones 1050-930 in Chino Avenue at Haven Avenue. The workunder this contract consists of construction of retaining wall, pilasters, metal fence andgate, grading, installation of pressure reducing station; installation of approximately 160linear feet of 12-inch diameter of CML&C steel recycled water mains; installation ofrecycled water appurtenances; connection to existing recycled water connections;approximately 50 linear feet of 12-inch PVC drain pipe; construction of concrete pad,installation of electrical and SCADA system; implementation of traffic control duringconstruction; and pavement repair and street restoration as described in the Plans andSpecifications for OMUC Contract No. UT1023.The Contractor will furnish all transportation, materials, tools, time, equipment, labor andsupplies to complete the improvements, together with the appurtenant work necessary orincidental to complete, in a workmanlike manner, the improvements as contemplated andas intended by the Plans and these Technical Specifications. The bidder must attach a pdf file to the electronic bid submission containing all of the completed and signed Bid Documents including a Bid Bond; all are labeled "BIDDER MUST INCLUDE THIS FORM AS PDF ATTACHMENT TO ELECTRONIC BID" at the top of the pages. A statement setting forth changes in other materials, equipment or other portions of the work including changes in the work prompted by the substitution shall be included. The burden of proof of the merit of the proposed substitution is upon the proposer. The City's decision of approval or disapproval of a proposed substitution shall be final. Cash, cashier's or certified check may be submitted in lieu of a bid bond. A personal or company check will not be accepted. This is a State Prevailing Wage Project. List all subcontractors being paid in excess of 1/2 of 1% of the total bid amount. Absent a named sub for any portion of the work, Bidder agrees he is qualified and will perform that portion of the work Bidders may not bid as a prime and also submit a bid as a subcontractor. It is the bidder's responsibility to become familiar with the contract documents in their entirety. All forms in the bid booklet must be properly completed to be deemed responsive.
