The work includes the furnishing of all labor and materials for the construction, complete in-place, of the following approximate quantities: 16 LF 15" RCP storm sewer 1 EA Relocated hydrant 8 VF Construct 2'x3' drainage structure 3 EA Clearing and grubbing 6500 SY Mill bituminous pavement (2" depth) 400 CY Common and Subgrade Excavation 80 CY Aggregate Base, Class 5 1100 SF 4" concrete median 1200 SF 6" concrete walk 950 LF B618 and B612 concrete curb and gutter 6700 SY Bituminous Mixture for Streets 3 EA Relocate lighting unit 1 LS Revise signal system 1 LS Traffic control interconnect system 16 EA Pavement messages, epoxy 1150 LF Pavement markings, epoxy 90 SF Sign Panels, Type C Together with structures, hydrants, valves, fittings, erosion control, restoration and related appurtenances. This project is subject to Prevailing Wage Rates. The provisions of Minn. Stat. 16C.285 Responsible Contractor are imposed as a requirement of this contract. All bidders and persons or companies providing a response/submission to the Advertisement for Bids/RFP of the City shall comply with the provisions of the statute. The bids must be submitted on the Bid Forms provided in accordance with the Contract Documents, Plans, and Specifications as prepared by Bolton & Menk, Inc., 3507 High Point Drive North Bldg. 1 Suite E130, Oakdale, MN 55128, which are on file with the City Clerk of Maple Grove and may be seen at the office of the Consulting Engineers. Each bid shall be accompanied by a bidder's bond naming the City of Maple Grove as obligee, certified check payable to the Clerk of the City of Maple Grove or a cash deposit equal to 5 percent of the amount of the Bid, which shall be forfeited to the City in the event that the Bidder fails to enter into a Contract.
