The proposal shall be submitted with the attached Bid Cover Sheet completed in its entirety. Warranty information, when applicable, shall also be included & submitted. All work to be performed during normal working hours. If bid price is over $10,000 or more, bidder must be signatory to relevant union, per terms of the Various Trades Contract and Project Labor Agreement. If bid price is over $75,000, you MUST include your MBE/WBE subs and relevant forms at time of bid. VCT (4,270 square feet) Main Office, conjoining offices, closets and bathroom Room 109 and conjoining offices Room 136, conjoining offices, and closets Admin office and hallway Scope of work: Remove all four layers of flooring down to beams VCT Sub floor Oak finish floor sub floor Remove old vinyl base Replace flooring with double 3/4 inch CDX plywood Install new 4 inch vinyl base Install VCT VCT provided by school VCT arranged in random pattern All loose furniture will be removed from areas by school Include new framing to allow subfloor to be properly fashioned Carpet (1,128 square feet) Counseling office Scope of work: Remove existing carpeting refashion and perform necessary sub floor repairs Removal of existing base Install of new 4inch vinyl base Install new carpet squares carpet squares provided by school Vendor will be responsible for cleanup of all areas worked in. Vendor will be responsible for supplying their own means of disposal and dumpsters. Vendor will be responsible for applying protection and finish to all new VCT tile installed by the vendor per the CPS new construction guidelines. 5 coats of finish in classroom/offices and 3 coats in corridors. Each Bidder must include in their Bid the premium cost for the performance and payment Bond in the full amount of their contract, when the project is over $50,000. If the Bidder fails to furnish the performance bond within seven (7) days after acceptance of the Bidder's Bid, by the Board, the Bid acceptance maybe rescinded and the Bidder maybe precluded from further Bidding with the Board. The performance Bond must be issued by a surety acceptable to the Board and Licensed by the State of Illinois.
