The project includes, but is not limited to, the partial structural reinforcement (4 oclock to 8 oclock) of approximately 450 feet of an existing 108 CMP culvert and the full circumferential structural reinforcement (12 oclock to 12 oclock) of approximately 60 feet of the CMP culvert, Debris Removal, Lawn Restoration, Erosion and Sedimentation Control, Preparation and Cleanup are included. Add Alternate Bid No. 1 Department of Public Works Yard Culvert Gunite Repair The project includes, but is not limited to, the partial structural reinforcement (4 oclock to 8 oclock) of approximately 65 feet of an existing 108 CMP culvert and the full circumferential structural reinforcement (12 oclock to 12 oclock) of approximately 40 feet of the CMP culvert, Debris Removal, Lawn Restoration, Erosion and Sedimentation Control, Preparation and Cleanup are included. Add Alternate Bid No. 2 Department of Public Works Yard Trash Rack Installation The project includes, but is not limited to, installation of a concrete collar and trash rack system at the upstream end of the stormwater culvert, Preparation and Cleanup are included. Prevailing wage rates will be required on this project. Proposals to receive consideration must be accompanied by a Certified Check or Bidder's Bond from a Surety Company authorized to do business in Pennsylvania, made to the order of the Historical Society of Mt. Lebanon in an amount equal to ten percent (10%) of the total amount of the Proposal as a guarantee that, if the Proposal is accepted, the successful Bidder will enter into an Agreement within fifteen (15) days after Notice of the Award of the Contract. Performance, Maintenance, and Labor and Material Payment Bonds, along with Public Liability and Property Damage Certificates of Insurance in the amounts specified, as well as Certificates of Workman's Compensation must be filed with the executed Agreement upon acceptance of the Proposal from the successful Bidder. The Commission of Mt. Lebanon, Pennsylvania reserves the right to waive any bid informalities or to reject any or all bids.
