In addition, the work is to be completed in accordance with the these specifications approved by the Allen County Highway Department's Executive Director, 200 East Berry St, Suite 280, Fort Wayne, Indiana 46802-2376. from Felger Road to O'Day Road from Arcola Road to US 33 from US 33 to Carroll Road The true intent ofthese specifications is to provide the completion in every detail ofthe improvement described herein and in these specifications. The Contractor for all or any part will furnish all labor, materials, tools, transportation, and necessary supplies, such as may be required under the terms oftheir contract, to make each part complete. Any deviation from these requirements must be stipulated in the contract. All proposals shall be made on FORM 96 (revised 2013), entirely completed, signed and notarized, with the Non-Collusion Affidavit signed and notarized, as required by law. A Bidder's Bond or Certified Check equal to 10% ofthe maximum bid must accompany each bid, along with an Affirmative Action Contract Compliance Statement and an Equal Employment Opportunity Policy Statement. The successfill bidder will be required to furnish a Payment and Performance Bond in the amount of 150% ofthe contract price, and Certificates ofInsurance covering Workmen's Compensation, Public Liability and Property Damage, and any other which may be required, before the contract can be signed and issued, or any work can be started.
