The City of Chattanooga is requesting quotes for the removal of existing carpet and installation of Luxury Vinyl Tile (LVT) to be installed at the Bessie Smith Hall Project Specifics o Luxury Vinyl Tile (LVT) -- Interface LVT Steady Stride Woodgrains B00103 -- Malt, see attached Specification Sheet or approved equal. To be laid in an Ashlar pattern. o Installation of flooring shall be in compliance with the manufacturers LVT Installation Instructions, attached and made part of this quote package. o All quotes shall include all labor, tools, supervision, all incidentals, etc., complete in place. Materials that are stated in the Quote Schedule Form 301-1, and shall be paid per units as measured in the field. o Rubber Base shall be 4" Flexco, Flextones 077 Driftwood, to include inside and outside corners, adhesives, etc. Complete-in-Place. o Floor Layout Plan Fl has been provided as part of this quote package. o Time is of the Essence for this project. Failure to deliver project by stated date, shall be subject to Liquidated Damages of $250.00 per calendar day. o Terms and Conditions are subject to City of Chattanooga Standard Procurement terms and conditions, unless otherwise stated. o The Contract shall require a Performance Bond 600-1, and a Payment Bond 601-1 attached, to be submitted by the selected vendor. o A Relative Humidity Test and a pH Test of the existing concrete slab have been scheduled for the week of November 16 and the results will be released via an addendum the week of November 23. o All questions shall be addressed to Debbie Talley at the email address bidinfo@chattanoogagov .
