CLARIFICATION DEADLINE 3/31/2021 12:00 PM FOB Delivered to the Township Building The Municipality is requesting prices for all materials to be delivered to the Municipality's Garage located at 1540 Sunbury Road, Pottsville, PA 17901. Bidders will need to contact the Municipality prior to making any deliveries so arrangements can be made for a Municipal Representative to be present to inspect the materials and assist in unloading if necessary. Supplier agrees to furnish and deliver those items for which prices have been indicated on the Schedule of Prices (Attachment 1) in accordance with the current PennDOT Specifications (Pub. 408), except bidders need not be prequalified by PennDOT (Sec. 102.01). It is understood that: (1) Bituminous materials will be purchased weight or converted gallons at 60 F. (2) Supplier must finish Form TR-465, "Daily Bituminous Mixture Certification." (3) The Municipality reserves the right to make an award on the basis of quotations received for any item or on the basis of the aggregate total for all like items on which quotations are received. The scope of the work is to construct Superpave Base Course and Subbase, including subbase proof rolling, tack coating and sealing of joint and backup material as indicated on Attachment 1. These items are incidental to placement of paving unless otherwise noted. - The Contractor is to remove excess material. - Complete all testing in accordance with PENNDOT Specifications Form 408 Section 409. - The Contractor will tack coat the vertical edge of the paving joint as per PENNDOT Pub. 408, Section 460. - The Contractor will provide a bill of lading and a signed CS-4171 for each type of material. - Saw cut or Milled Paving Notch (RC-28M) is required and is incidental to paving. - Tack Coat is required per Section 460 of PennDOT Specifications Form 408 and is incidental to paving. - The Contractor will saw cut the paving notch and excavate out the subbase material needed to provide a proper overlay transition as per PENNDOT Pub. 72M, Standards for Roadway Construction, RC-28M. - Milling of driveway notches are to be done as directed and in a fashion that maintains the shoulder slope and provides a smooth transition from the driveways to the shoulder. - The Contractor will supply the TOWNSHIP with Bituminous Mix Design which meets PennDOT Specifications Form 408 prior to receiving the Notice to Proceed. - Bituminous Delivery tickets must contain the Job Mix Number (CAMMS No.) from the appropriate bituminous design. - Bituminous Material Certifications must contain the Job Mix Number (CAMMS No.) from the appropriate bituminous design. - The Contractor will place the specified type and depth of material (all depths are compacted in place) indicated in the Schedule of Prices. This material will conform to PENNDOT Pub. 408, Section 409 except Marshall Testing that is not required. The Contractor will provide delivery tickets and a daily bituminous material certification on a TR-465 or an approved CS-4171 form for each day's placement of bituminous material. - The Contractor will seal joints using materials specified in PENNDOT Pub. 408, Section 409.3. - Backup material (topsoil in lawn areas and subbase material in other areas) is incidental to wearing course. - Retain existing pavement cross slope unless otherwise noted.
