Affidavit of Compliance Affidavit of Experience Bid Bond - 10% of Total Bid Amount Signed Offer to Contract Prevailing Wage Applies (DuPage County Rates) Due Before Award Per Letter of Intent: Certificate of Insurance (with signed endorsements) Payment and Performance Bonds Substance Abuse Prevention Program Due within thirty (30) days of Award: Contractors Safety Checklist (before work begins) The City of Naperville is accepting bids from experienced and authorized vendors to provide the following work: Part 1: The Work to be performed under this Contract consists of the replacement of existing control valves, butterfly valves, pressure gauges, pressure transmitter, sump pumps, flow meter and associated piping in the PSA 15B valve vault. The work also includes removing existing control panel, installation of new PLCs and enclosure, replace the water supply and drainage pipes for three chlorine analyzers, replacement of all current lights and LED light fixtures and installation of associated electrical wires and new conduits. PAS 15B vault and Well 16 pipes, valves, walls, floors and ceiling painting. Part 2: All work associated with Pump 2 & 3 discharging line. Replace the automatic butterfly valve and solenoid valves with check valve and butterfly valve. Replace existing customized pipe reducer. Replace the wires, install new conduit as required by code.
