The Project con-sists of the total reconstruction of Glen view, Park view, Lawn dale,Eastwood and Silver brook Aven-ues in the Monroe Manor Subdivi-sion in the City of La Porte. The project will be bid with a base bid containing several of the streets.The remaining streets will be bid as alternates separated per street to allow for award based on fund-ing available. The total project contains the reconstruction of upto approximately 4,000 lineal feet of roadway including the installa-tion and or replacement of ductile iron water main, gravity storm and sanitary sewers, sanitary and wa-ter services, concrete curb andgutter, concrete sidewalk, con-crete aprons, asphalt paving,grass restoration, tree plantings and all other work within the Cityof La Porte. Bids will be received for a singleprime Contract. Bids shall be on a unit price basis. Two (2) cop-ies of the sealed Bids bearing the project name as well as the name and address of the bidder shall be submitted to Courtney Part hun City of La Porte - City Clerk, 801 Michigan Ave, La Porte, IN 46350.Bidding documents will be avail-able on April 16th, 2021. A certified check, cashier's checkor bid bond in an amount equal tofive percent (5%) of the total amount of the bid will be required with each bid as security for theproper execution of the contract. A 100% Labor, Material, and Per-formance Bond will be required of successful bidder. Bidder shall submit Indiana State Form96, the City of La Porte Affidavit of Com-pliance and be certified to dowork in the State of Indiana and the City of La Porte.
