County: Weber This Project Requires Using the Electronic Certified Payroll Program. See Attachment to Notice to Contractors. This Is a P+t (Price + Time) Project. Wage Rates: Apply Engineer's Estimate: Greater than $1.5M but less than or equal to $3M (Qualified Health Insurance required) The principal items of work are as follows (for all items of work see attachment): Bonded Wearing Course - Type B Mobilization Reconstruct Cleanout Box The project is to be completed: to be determined by competitive bid Project information can also be reviewed at the main office in Salt Lake City, its Region offices, and its District offices in Price, Richfield, and Cedar City. As required, a contractor's license must be obtained from the Utah Department of Commerce. A prime contractor is subject to qualified health insurance requirements if the prime contract is in the amount of $2,000,000 or greater. A subcontractor is subject to qualified health insurance requirements if the subcontract is in the amount of $1,000,000 or greater.The Contractor shall demonstrate compliance with qualified health insurance requirements at the time of the execution of each initial prime contract Each bidder must submit an electronic bid bond from an approved surety company using UDOT's Electronic Bid System (EBS); or in lieu thereof, cash, certified check, or cashier's check for not less than 5% of the total amount of the bid, made payable to the Utah Department of Transportation, showing evidence of good faith and a guarantee that if awarded the contract, the bidder will execute the contract and furnish the contract bonds as required. The right to reject any or all bids is reserved.
