Contract A - Elevated Storage Tank is located at 1459 Prospect Street, Weathersfield Township, Trumbull County, Ohio. This portion of the project will include the following items: a.Installation of the Elevated Storage Tank and various site improvements Contract B - The Water Booster Station is located at 3620 Main Street, Weathersfield Township, Trumbull County, Ohio. This portion of the project will include the following items: a.Connection to 20" Water Transmission Main owned by MVSD. b.Installation of approximately 2,750 lineal feet of 12" C909 PVC Water Main. c.Installation of approximately 450 lineal feet of Horizontal Directionally Drilled C900 Fusible PVC Water Main. d.Six connections to existing water mains. e.Installation of a new Water Booster Station with site improvements. f. Removal of an existing water meter and abandonment of structure. Contract A = $2,200,000.00; Contract B = $1,550,000.00 The project will be funded by a loan from the OPWC, project number CF03V. All contractors and subcontractors involved with the project will to the extent practicable use Ohio products, materials, services, and labor in the implementation of their project. Additionally, contractor compliance with the equal employment opportunity requirements of Ohio Administrative Code Chapter 123 is required. Each Bidder must furnish a Bond, signed by a Surety Company authorized to do business in the State of Ohio, in the sum of 100 percent of the amount bid, or a Certified Check, Cashier's Check, or Letter of Credit, revocable only by Trumbull County, equal to 10 percent of the amount bid. Such Bond or check is a guarantee that if the Bid is accepted, a Contract will be entered into and the performance of it properly secured by a Performance and Payment Bond in the amount of 100 percent of the Contract (unless a 153.571 bond is provided with the bid), in accordance with Section 153.54 of the Ohio Revised Code. Attention is directed to all bidders that a Non-Collusion Affidavit must be signed. The successful Bidder must sign a statement concerning Personal Property Taxes, in accordance with Ohio Revised Code, Section 5719.042. Attention is directed to all bidders that Trumbull County adopted a "New" Drug and Alcohol-Free Workplace Policy on September 7, 2016, and terms of this Policy also apply to any Contractor[s] directly reimbursed for services through the County; a copy of the Policy is to be part of proposal; and that each proposal be accompanied by a "SignOff Sheet" duly signed by the bidder thereof. Attention is directed to all bidders that a Safety Statement Affidavit must be signedAttention is directed to all bidders that a Non-Collusion Affidavit must be signed. Attention is directed to all bidders that a Safety Statement Affidavit must be signed A Contract shall be awarded to the lowest and best Bidder as soon as practical after Bid opening, subject to the reservation of the preceding paragraph. The successful Bidder must enter into a Contract with Trumbull County within 10 days after receipt of the formal "Notice of Award." No bidder may withdraw his bid for a period of 90 days after the time of opening bids. Federal prevailing wages are required for this project. Substantial Completion: 540 Days from Notice to Proceed Restoration and Ready for Final Payment: 45 days from date of Substantial Completion Liquidated damages at $1,000.00/calendar day shall apply after the Substantial Completion date. Restoration and Ready for Final Payment Date shall be forty-five (45) days after date Substantial Completion was scheduled or adjusted by Change Order, or earlier if actual date. Liquidated damages after Restoration and Ready for Final Payment Date of $250/day shall apply. Liquidated damages are cumulative if damages from Substantial Completion and Ready for Final Payment overlap. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
