The City of Crown Point, Indiana through its Board of Public Works and Safety (City of Crown Point) is requesting proposals for the demolition of the building improvements located at 812 E. Monitor Street, Crown Point, IN 46307 as part of its determination that the structure is an unsafe building pursuant to Indiana Code 36-7-9. Contractors providing proposals for the project shall include: Demolish, remove and properly dispose of all building materials and provide written proof of proper disposal of the same and properly grade soil to be brought to level grade with surrounding elevations. All Contractors submitting proposals must provide a total project cost for completion of the work at the forms supplied by the. Proposers are invited to submit additional information and qualifications defining the detail of their Proposal. The total project cost must include the necessary contracting permits, licenses, labor, equipment, tools, insurance, project services costs and so forth incidental to and required to fully execute the work. The successful Proposer shall submit its Insurance Certificate naming the City of Crown Point and its professional service agents as additionally insured prior to the start of the work. The successful Proposer shall commence demolition work upon receipt of the notice to proceed by the City of Crown Point and shall complete the work within 30 day. The successful Proposer shall be responsible for the coordination of work with other contractors as needed. The City of Crown Point reserves the right to reject proposals with or without cause and for any reason, to waive any irregularities or informalities, and to solicit and re-advertise for other proposals and to award the proposal to the lowest responsible and responsible Proposer. Incomplete or non-responsive proposals may be rejected by the City of Crown as non-responsive or irregular. The City of Crown Point reserves the right to reject any proposal for any reason, including, but without limitation, if the Proposer fails to submit any required documentation, if the Proposer is in arrears or in default upon any debt or contract to the City of Crown Point or has failed to perform faithfully any previous contract with the City of Crown Point or with other governmental jurisdictions. All information required by this Request for Proposals must be supplied to constitute a proposal. Each Proposer must ensure that all employees and applicants for employment are not discriminated against because of race, creed, color, sex or national origin. Each Proposer and any subcontractors shall be in compliance with the City of Crown Point's Responsible Bidding Ordinance 2008-01-16. Each Proposer shall participate in the E-verify program according to statute IC 22-5-1.7 et seq. Each Proposer must certify that it has not participated in collusion or other anticompetitive practices in connection with its RFP by executing and returning with its RFP Response a Non-Collusion Affidavit. As required by IC 5-22-16.5, the Proposer must certify that it is not engaged in investment activities in Iran. Providing false certification may result in the consequences listed in IC 5-22-16.5-14.
