Clinton Ave, Fresno CA Description: Contractor shall provide all supervision, labor, materials, services, tools, equipment, and transportation for design and construction to upgrade OR #4 into an Interventional Imaging Surgical Suite in accordance solicitation, Scope of Work and applicable attachments. IAW VAAR 836.204, Disclosure of the magnitude of construction projects, the magnitude of construction for this project is between $2,000,000.00 and $5,000,000.00. Note #1 to OFFERORS: This RFP is being issued for Full and Open Competition. Note #2 to OFFERORS: This project will be awarded IAW FAR 36.3 - Two-Phase Design-Build Selection Procedures. Note #3 to OFFERORS: Bid Bonds, Payment Bonds and Performance Bonds shall be required under this solicitation and any resultant contract. Scope is defined as work or activities related, or closely related, to that which is outlined in the Statement of Work such as, but not limited to, demolition and installation required for upgrade of an operating room, to include design, finishes work, electrical work, plumbing work and other utilities work in a hospital environment.. Recency is defined as a project completed (100%) within the past three (3) years of the proposal issue date of the RFP. Relevancy is defined, for purposes of this evaluation, as design-build repair, alteration, installation and related demolition of existing infrastructure completed (100%). Infrastructure is defined repair/alteration of a healthcare facility or improvements to a healthcare facility, such as utilities. Repair, alteration and related demolition include repair and/or modernization of an existing healthcare facility and replacement or upgrade to an existing healthcare infrastructure. Questions of a technical nature must be provided in writing, and shall be submitted by the prospective offerors, to James D. W. Rall. Questions shall be submitted only via e-mail, using the REQUEST FOR INFORMATION (RFI) form (see attachments) to: james.rall@va.gov.
