HVAC upgrades including, but not limited to, replacement of two (2) Multi-Zone Roof Top Units, addition of a second boiler for system redundancy, related duct/piping/electrical/structural revisions as required to complete system, connections to building automation system and commissioning of new equipment at Centennial Elementary School. Contract Type: Single prime lump sum contract. Stipulations: Bidder will be required to provide Bid Security in the amount of not less than five percent (5%) of the base bid amount proposed, in the form of a certified check or cashier's check made payable to the Owner, or a Bid Bond. The successful Bidder will be required to furnish and pay for a satisfactory Performance and Payment Bond in the full amount of the contract. The successful Bidder shall be required to comply with all provisions of the acts of the General Assembly of the State of Illinois related to Wage Rates, Discrimination, Sexual Harassment, Alcohol Use, Preference to Illinois Workmen, Fair Employment Practices Commission, and Equal Employment Opportunity. The successful Bidder will be required to furnish and pay for a criminal background investigation for all employees involved in the project. The background investigation will consist of two parts, as follows:
Part 1: Completion of a "Uniform Conviction Information Act Name Inquiry" with the Streator Police Department for each employee.
Part 2: Completion of the "Criminal Background Certification" form included in Project Manual (Section 00 43 00) for each employee.
The successful Bidder shall be required to comply with the Substance Abuse Prevention on Public Works Act (Public Act 95- 0635). The successful Bidder shall not pay less than the prevailing rates of wages as determined by the Illinois Department of Labor to all laborers, workmen, and mechanics performing work under this contract, and shall comply with the requirements of the Illinois Prevailing Wage Act (820 ILCS 130/1-12). All Contractor's bonds shall include a provision as will guarantee the faithful performance of such prevailing wage clause as provided by this bid specification or contract.
