Directions to site: From the Charleston Area: Take I-64 W to Exit 34 CO Rd 19 Hurricane. Turn left on CO Rd 19. Continue 0.4 mile turn right on Midland Trail/Teays Valley Rd. Continue 1.5 miles and turn left on US 60 E. Continue 0.1 mile and turn right on State Rte 34 S. Continue 15 miles and turn right on State Rte 3 W. Continue 7.4 miles to West Hamlin. The property is located at the junction of Rte 3 and Bias Fork Rd on the left. Approaching from the Huntington Area: Take I-64 E to Exit 11 Hal Greer Blvd/Downtown. Turn right onto Rte 10 S. Continue 12 miles turn right on Madison Creek Rd. Continue 1.2 miles turn left on Dairy Rd. Continue 2.2 miles turn left on Rte 10 N. Continue 0.4 miles turn right on Rte 3 to West Hamlin. The property is located at the junction of Bias Fork Rd and Rte 3 The Contractor, the Contractor's agents, representatives, and subcontractors ("Contractor") shall provide all equipment, materials, labor, and professional services for the plugging of the oil and/or natural gas well(s) identified herein, and the reclaiming of all land disturbed in association with those plugging operations in accordance with West Virginia Code 22-6-23, 22-6-24, and all rules promulgated thereunder. Well detail: The Office of Oil and Gas has no well records for the Milton Clay #1, API# 47-039-03599. Accordingly, the well characteristics and plugging work order will be an estimation of down-hole conditions from the reference well, API# 47-043-00299 (see Section VII. Appendix A). The estimated well depth is 3,000 ft. The wellhead currently has 4 1/2 - inch casing that extends above the ground surface 30 inches. There is 2-inch tubing sleeved through the 4 1/2 - inch casing on a hanger to an estimated depth of 1,600 ft. and set in a 4 1/2 - inch gas packer. The 2-inch tubing is connected at the well head to a 2-inch buried production line, which will need to be disconnected and plugged below surface. There is estimated to be 30 ft. of 10-inch conductor, 200 ft of 6 5/8- inch and/or 8 1/4 -inch surface casing and 1,605 ft of 4 1/2 -inch intermediate casing with 2-inch tubing on 4 1/2 -inch packer to TD (approx. 3,000 ft). The well appears to be open hole below the 4 1/2 -inch casing. The amount of cemented casing is unknown. There is no reported fresh water or coal from the reference well. There is a saltwater show at 800 ft. The well head currently has 125 psi inside the 2-inch tubing. The producing formation is likely the Berea
