Bidders must complete and submit the Small Business Affidavit (Form No. 275-000-01) for approval prior to receipt of bid documents. The form may be accessed at: https://www.fdot.gov/equalopportunity/bdi.shtm It is only necessary to submit the Small Business Affidavit (SBA) form once per year.
