Performance Bond and Payment Bond in the amount of 100 % of the contract price The bids shall be lump sum and shall be received for a prime contract. Contractor is to include an allowance of $3500.00.00 in the bid. Allowance is to be used for changes in scope resulting from unforeseen conditions at the site. Allowance can only be used with written approval by Architect and Owner. Any unused allowance must be returned to the Owner. The following documents must be submitted with the bid: Completed State Bid Form 96 (revised 2013) including the Non-Collusion Affidavit Certificate of Qualification from Indiana Public Works Certification Board Statement of Company Equal Employment Opportunity and Non-Discrimination Policy Indiana Legal Employment Declaration (E-Verify) Iran Certification Written plan for employee drug testing program W-9 (In the absence of a Federal Identification Number, a Social Security Number must be provided.) The successful bidder(s) will be re- quired to furnish insurance covering Workmen's Compensation, Public Liability and Property Damage and any other which may be required, before the contract can be signed and issued.
