Site work and renovation of a medical facility in Lafayette, Indiana. Completed plans call for the renovation of a one-story above grade medical facility; and for site work for a medical facility.

Bids may be withdrawn prior to such time, but no bids shall be withdrawn for a period of 60 days thereafter. The Board of School Trustees of the Tippecanoe School Corporation, Tippecanoe County, Indiana reserves the right to retain any and all bids for a period of not more than sixty (60) days and said bids shall be and remain valid and in full force and effect during said time. The Owner reserves the right to reject any and all bids and to waive any informalities, irregularities, or errors in the bidding to the extent permitted by law. This includes the right to extend the date and time for receipt for bids. A general description of the work for which proposals are to be received consists of: Interior renovation of an existing single-story slab-on-grade wood-framed with partial basement dentist office into a new Corporation Wellness Clinic, including limited exterior facade repairs, sitework, and landscaping to support the interior renovations. All work shall be in accordance with the Bidding Documents, including the Specifications and the Construction Drawings. Bids shall be completely and properly executed on Bid Form 96 (Revised 2013), as prescribed by the Indiana State Board of Accounts, giving financial data as recent as possible and in no event more than 90 days old. Each bid shall be accompanied by an executed noncollusion affidavit, an executed bid bond in the minimum amount of five percent (5%) of the bid and such other documents required by the Instructions to Bidders. Bid bond may be a certified or cashier's check payable to the Board of School Trustees of the Tippecanoe School Corporation, Tippecanoe County, Indiana, or an acceptable bidder's bond. Should a successful bidder withdraw their bid, or fail to execute a satisfactory Contract, the Owner may then declare the bid deposit forfeited as liquidated damages. Requirements for signing bids: 1. Any bid not signed by the individual making same shall have attached to it a power of attorney evidencing authority to sign the bid in the name of the person for whom it is signed. 2. A bid signed for a partnership shall be signed by one of the partners or by an attorneyin-fact. If signed by an attorneyin-fact, there shall be attached to the bid a power of attorney, evidencing authority to sign the bid, executed by the partners. The successful bidder will be required to furnish a satisfactory Performance Bond and Payment Bond in an amount equal to one hundred percent (100%) of the total contract price. The Instructions to Bidders and General Conditions of the Contract for Construction contained in the Project Manual Specifications for the Project are by this reference made a part hereof, and all bidders shall be deemed advised of the provisions thereof and of the General Conditions, Specifications, and Drawings for the Project.
