Bidder (prime contractor) must have, at time of bid submittal and for the duration of the contract, a valid, current: Contractor License(s): a HAZ - Hazardous Substance Removal Certification from California Contractor State License Board (CSLB). Hazardous Materials Certificate of Registration issued by U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration. Hazardous Materials Transportation License issued by California Highway Patrol (CHP). Trauma Scene Waste Management Practitioner (from State of California Department of Public Health, Medical Waste Management Program). Waste Tire Hauler Registration issued by Cal Recycle. Hazardous Waste Operations and Emergency Response (HAZWOPER) Certificate of 40 hours of training. Hazardous Waste Transporter Registration issued by the California Department of Toxic Substances Control. Confined Space Entry Certification, as required by the Department of Industrial Relations, Division of Occupational Safety and Health (Cal/OSHA), in Exhibit A, Section 17.
