In submitting a quote, each Bidder thereby agrees that, if awarded a contract, Bidder shall execute a standard agreement, a sample of which is attached to the RFQ package for reference. ($1,000,000) Automobile Insurance, coverage shall include owned, hired, non-owned, OR any auto. ($1M each incident) Pollution with Additional Insured Endorsement, shall include Completed Operations and be on a primary and non-contributory basis. ($500K aggregate; $250K each occurrence) Crime or CRIME BOND or FIDELITY BOND with Additional Insured Endorsement, must name the LACDA as Loss Payee OR provide copy of Client Coverage Endorsement - shall cover EMPLOYEE theft and/or forgery. ($50K each occurrence) Evidence of a valid B-1 General Contractors license. EPA Certification. (If applicable) SCE/SoCalGas Energy Upgrade California Participating Contractor.
