Renovation of a municipal facility in Fort Wayne, Indiana. Completed plans call for the renovation of a municipal facility.
Contractors and suppliers must conform to IC 5-16-13. Be sure to familiarize yourself with these changes before bidding on public works projects. The bids shall be lump sum and shall be received for a prime contract. Contractor is to include an allowance of $75,000.00 in the bid. Allowance is to be used for changes in scope resulting from unforeseen conditions at the site. Allowance can only be used with written approval by Architect and Owner. Any unused allowance must be returned to the Owner. Contractors working on public works projects involving the construction, reconstruction, alteration, or renovation of a public building must be prequalified through the Public Works Certification Board if project is estimated to be greater than $300,000.00. (IC 5-16-13-10 and 4-13.6-4) The bid shall also be accompanied by a bid bond in the amount of 10% of the bid. The bid bond must be present and attached to the original bid. Bid envelope (minimum size 9" x 12") must be sealed and include company name and address, project name, bid reference number, and date and time of bid opening. Each bidder is required to include one (1) original and two (2) copies of their bid. The following documents must be submitted with the bid: Completed State Bid Form 96 (revised 2013) including the Non-Collusion Affidavit Statement of Company Equal Employment Opportunity and Non-Discrimination Policy Indiana Legal Employment Declaration (E-Verify) Iran Certification Written plan for employee drug testing program Certificate of Qualification from Indiana Public Works Certification Board W-9 (In the absence of a Federal Identification Number, a Social Security Number must be provided.) Required documents must be included with each submittal. Contractors and suppliers shall conform to IC 5-16-13. The successful bidder(s) will be required to furnish insurance covering Workmen's Compensation, Public Liability and Property Damage and any other which may be required before the contract can be signed and issued. A Performance Bond and Payment Bond in the amount of 150 % of the contract price shall be required upon notification of the successful bidder prior to signing of the contract. Bids shall not be modified, withdrawn or canceled, without the Owner's written consent, for a period of Sixty (60) calendar days commencing from the day bids are received. An out of state contractor shall have Indiana State Gross Income Tax withheld by Allen County, unless he can show proof that he is registered with the Indiana Secretary of State to conduct business in the State of Indiana. No fee will be charged for permits required and issued by the Allen County Building Department for work done on any Board of Commissioners of the County of Allen, Indiana owned property. Documents may be available for inspection at the Allen County Department of Public Purchase, 200 East Berry Street, Suite 495, Fort Wayne, Indiana 46802 The Board of Commissioners of the County of Allen, Indiana reserves the right to reject any and all bids for failure to comply with applicable laws and/or with the Notice to Bidders and reserves the right to waive irregularities.
