The project consists of furnishing all labor, materials and equipment to perform to perform a complete Mechanical Integrity testing (MIT) on the industrial injection well located at the South Miami Heights Water treatment plant and then internal integrity testing of three (3) existing aquifer storage and recovery wells at the West Wellfield. The work consists of mobilizing to the South Miami Height plant and performing a complete MIT on the Industrial injection well that has a final FRP casing of 16.37- inch ID down to 2,770 feet bls. That consist of suppressing artesian flow, removing wellhead, scrubbing the casing, flushing the well with potable water, performing a video survey for the entire length of the casing and open hold, then performing a packer pressure test, logging for temperature and conductivity, finally performing a Radioactive Tracer Survey and reassembling the wellhead and purging the well to background conditions using a holding tank of 90,000 gallons and pumping the water into the sewer located onsite. Then the contractor can move to the West Wellfield ASR wells. The work includes suppressing the artesian flow with a food grade salt brine, removing the existing well head, scrubbing the 29 inch ID casings with a wire brush, flushing with potable water, doing a television survey of the entire casing while being able to see the joints, performing a pressure test up to 150 psi on each well casing, and rebuilding the well head providing new bolts and gaskets, finally purging each well to return to background water quality conditions using a 90,000 gallon tank and then using a tanker truck to remove the purge water offsite. Automobile Liability Insurance covering all owned, non-owned and hired vehicles used in connection with the work, in an amount not less than $300,000 combined single limit per occurrence for bodily injury and property damage. 7360 RPQs are NOT SBE-Con 100% Set-aside solicitation, however the RPQ may be assigned a SBE-Con Trade set-aside and goal. The SBE-Con Trade-aside and goal if applicable will be will be stipulated on the RPQ and the Invitation to Bid or in the Projects Solicitation Documents. All prime contractors submitting a bid for RFQ/project with a small business measures (s) must submit the small business development "certificate of assurance" form properly completed, signed and notarized with their bid document at the time of bid submittal. Failure to submit the required certificate of assurance form at the time of bid submission shall render the bid non compliant to the contract requirement and section 10.33.02 of the code of miami-dade county 7360 RPQs Federally Funded may be subject to the Disadvantaged Business Enterprise (DBE) Program. The DBE goal will be stipulated on the RPQ and the Invitation to Bid or in the Projects Solicitation Documents. 7040 and 7360 RPQs with an estimated project value in excess of $700,000.00 may be assigned a Small Business Enterprise Goods (SBE-G) or Small Business Services (SBE-S) program goal. The SBE-G or SBE-S goal if applicable will be will be stipulated on the RPQ and the Invitation to Bid or in the Projects Solicitation Documents. All RPQs with an estimated project value $100,000 or above are subject to Responsible Wage Rates. The wage rate will be stipulated on the RPQ and the Invitation to Bid or in the Projects Solicitation Documents.
