Prequalification of Horizontal Construction Forms Mass.gov. these forms are to be completed and submitted to the Pre-qualification office for approval Contractor Qualification Highway Division The work under this contract consists of modifications to the pavement markings and traffic signage at the intersection of Essex Street / Jackson Street / Lincoln Avenue (also known as Cliftondale Square) in Saugus to result in a roundabout configuration. The work is intended to improve the functionality, capacity, and safety characteristics of the intersection and the corridors leading to the intersection for all users. All work under this contract shall be done in conformance with the Standard Specifications for Highways and Bridges dated 2022, the 2017 Construction Standard Details, the 1990 Standard Drawings for Signs and Supports; the 2009 Manual on Uniform Traffic Control Devices (MUTCD) with Massachusetts Amendments and the Standard Municipal Traffic Code; and these Special Provisions.
