The Contractor shall provide all labor, equipment and incidentals necessary for the renovation of one home in accordance with the specifications and drawings under the Bureau of Indian Affairs (BIA), Housing Improvement Program Guidelines. The location of the home is 19010 CR 1280, Gracemont, OK 73042 (Caddo County). Approximately four (4) miles southwest of Gracemont, OK, at the "T" intersection of County Road 1280 and County Street 2630, go east. House is the first brick house on the left/north. In accordance with FAR 36.204, Disclosure of the magnitude of construction projects, the range for this project is between $25,000 and $100,000. This requirement will be awarded as a firm-fixed price contract. Performance and payment bonds and alternative payment protections are required for this award. A bid guarantee is required and shall be submitted with offer. The bid guarantee amount shall be at least 20 percent of the offer price. The offeror shall submit the following: 1.) SF-1442 form completed and signed to include DUNS or Unique Entity ID 2.) Technical capability to perform requirement 3.) Past Performance containing information regarding similar construction contracts and award amount 4.) Pricing and schedule of values 5.) Bid guarantee 6.) Complete highlighted Representations, Certifications and Other Statements of Bidders in Section K Failure to submit any of the above mentioned documentation will automatically eliminate the offer.
