Unsheltered Resident Encampment Site Waste Removal and Cleanup Services in East Los Angeles County. Bidder shall possess at the time of bid submittal and for the duration of the Agreement, a valid and current Motor Carrier Permit issued from the California Department of Motor Vehicles (DMV) as described in Section 9 of this Invitation for Bid and in Exhibit E of the Proposed Forms of Agreement, Attachment 13. Bidder and any subcontractor (for pest control) shall possess at the time of bid submittal and for the duration of the Agreement, a valid and current registration with the Department of Industrial Relations (DIR). The Prime Contractor shall maintain the following training, permit, and licenses for the duration of this Agreement: Contractor License(s): Class A General Engineering license and a Hazardous Substance Removal (HAZ) Certification from California Contractors State License Board (CSLB). Hazardous Materials Transportation License issued by California Highway Patrol (CHP). Hazardous Waste Transporter Registration issued by California Department of Toxic Substances Control. Hazardous Material Certificate of Registration issued by U.S. Department of Transportation Pipeline and Hazardous Material Safety Administration. Trauma Scene Waste Management Practitioner (from state of California Department of Public Health, Medical Waste Management Program). Waste Tire Hauler Registration issued by Cal Recycle. Hazardous Waste Operations and Emergency Response (HAZWOPER) Certificate of 40 hours of training. Motor Carrier Permit issued by California Department of Motor Vehicles (DMV). Confined Space Entry Certification, as required by the Department of Industrial Relations, Division of Occupational Safety and Health (Cal/OSHA), in Exhibit A, Section 17. A permit and contractors program documents will be required by the start of work. The Prime Contractor or Subcontractor shall maintain the following training, permit, and licenses for the duration of this Agreement: Contractor or subcontractors applying or spraying disinfectant shall possess a Pest Control Business (PCB) license and have a Qualified Applicator License (QAL) with Category K (health related) staff to oversee the business operations. elations (DIR). Contractor or subcontractors applying or spraying disinfectant shall also be registered with each individual county agricultural commission where the disinfectant will be applied. Proof of these requirements shall be provided to Caltrans Contract Manager prior to applying or spraying disinfectant.
