Question End Date 12/14/2022 12:00 PM Anticipated Award Date 2/14/2023 The purpose of this Request for Proposal (RFP) is to solicit competitive sealed proposals for The School Board of Citrus County, Florida, (hereinafter referred to as CCSB) from qualified firms who are approved by the Universal Service Administration Company (USAC) for the purchase and installation of Category 6A cabling at multiple District locations as required in support of new wireless access points in full compliance with the E-Rate Program. CCSB is currently seeking pricing for the purchase and installation of Category 6A Cabling at seven (7) District locations. This project requires Category 6A cable to be terminated from Category 6A Patch Panel to Surface Mounted Category 6A ports to fully realize the performance potential of the new access point models. The submitted Proposal will be for the equipment and labor costs associated with running Category 6A network cable from multiple IDFs through J-hooks to requested endpoint locations throughout seven (7) school sites within the Citrus County School District. Respondents shall refer to the Proposal Price Form - Attachment A for all equipment and labor needed. If Respondent proposes a product other than specified, it is the Respondents responsibility to submit, with the proposal, samples, descriptive literature and/or detailed specifications which illustrate the products ability to meet the technical specifications required. Proposals received without this information, or with insufficient information, as determined by CCSB, may not be considered. Respondent Minimum Qualifications: A. Respondent should have the ability to secure all inventory by April 1, 2023, or as soon as possible thereafter, and to complete ALL services of this contract by August 9, 2023. B. Respondent shall have a minimum of five (5) years of experience as a data communications contractor for commercial, industrial, and instructional customers successfully performing projects of similar scope and size and operating under the same legal entity name within the State of Florida, as verified by sunbiz.org prior to Proposal opening. C. Respondent shall have and maintain all applicable Florida State Licensing, Business Tax Receipt, Installer Certifications, etc. necessary to perform the services of this Contract. Respondent shall provide copies of all applicable documents with proposal. D. Respondent shall meet all Federal E-Rate requirements outlined under Special Conditions herein and provide with their proposal (a) proof of a valid Service Provider Identification Number (SPIN), (b) proof of a valid Federal Communications Commission Registration Number (FRN), and (c) evidence of FCC Green Light Status, to be verified by CCSB through the USAC website. E. Respondent shall provide with their proposal descriptive literature and/or detailed specifications for any alternate equipment or product proposed which illustrates the products ability to meet the technical specifications required. Proposals received without this information, or with insufficient information, as determined by CCSB, may not be considered. F. Respondent shall have sufficient financial support, tools, equipment, organization, facilities, delivery method, and personnel to ensure that they can satisfactorily execute the services if awarded the contract under the terms and conditions stated herein. G. Respondent shall complete the enclosed forms that the CCSB may use to evaluate the Respondents capabilities to perform the work as specified. Provide at least three (3) commercial/governmental references from agencies which Respondent has provided goods or services for projects similar in scope and size. At least two (2) reference should be a public school system. H. Respondent shall provide evidence of required insurance coverage or proof of insurability in the amounts specified with their proposal.
