Cities: Galax, Martinsville, Radford, City of Roanoke, and Salem. Counties: Carroll, Craig, Floyd, Franklin, Giles, Henry, Montgomery, Patrick, Pulaski, and Roanoke County. The contractor shall furnish all labor, supervision, equipment, tools, parts, supplies and materials, as necessary, to perform the services as described herein: 1) Construct a private sewer line connecting the subject property to the Henry County Public Service Authority (HCPSA) public sewer line in accordance with the Virginia Uniform Statewide Building Code (VUSBC), and all other applicable state and local laws, regulations, and ordinances. 2) Within 24 hours of signature of agreement to construct the sewer line, the contractor shall install approximately 10 feet of new 4-inch Schedule 40 sewer line from the house foundation to the existing septic tank to relieve the current system back up from a collapsed sewer line. 3) Within 48 hours of signature of agreement to construct the sewer line, the contractor shall have the contents of the existing 750 gallons septic tank serving the subject property pumped by a properly licensed sewer hauler. The contents of the existing septic tank shall be continuously pumped every 10 days thereafter, until connection to the HCPSA public sewer line is complete. 4) Serving as an agent for the property owner, the contractor shall contact HCPSA and fill out all necessary applications to initiate HCSPA's extension of a sewer tap to the edge of the Virginia Department of Transportation's Right of Way at 4606 The Great Road, Fieldale, Virginia 24089. Initiation of HCSPA extension shall include payment of the $1,750 connection fee. 5) Upon HCSPA completion of sewer tap installation, contractor shall install a 4 inch private sewer line in compliance with the VUSBC from the point the sewer exists the subject properties house foundation to the HCSPA tap. The 115-foot private sewer line will begin at an approximate depth of 18 inches exiting the house foundation, and terminate at approximately 36 inches depth at the HCSPA tap. 6) Upon completion of the sewer connection, the existing septic tank shall be pumped by a properly licensed sewage hauler, the tank crushed in place, lime placed over the crushed tank, and the tank hole filled with clean backfill material. 7) Follow all regulations and permitting pertaining to erosion and sediment control including stabilizing, seeding and grading the site after construction to return to the original state. Control construction runoff with proper practices so as not to become a nuisance to the owner or to neighboring properties or cause sediment to be discharged into state waters and drainage ditches. Any construction debris must also be removed from the site and disposed of properly. 8) Comply with all requirements of the Department of Professional and Occupational Regulations (DPOR) for contracting and executing the contract with the Virginia Department of Health. Operate within the requirements of the DPOR Board for Contractors.
