Submission Requirements: Submission Form (Appendix E) Response content for rated criteria (Appendix C) Provide proof of current professional liability (errors and omissions) coverage as referenced in Appendix A, Schedule 2-B. Provide proof of current commercial general liability coverage as referenced in Appendix A, Schedule 2, 8.02. Submit a valid clearance letter from the Workers' Compensation Board (WCB - Alberta) stating that the Respondent has an account in good standing with the Board. Contractor to provide a completed Proof of Vaccination Declaration Form included as Attachment 3.
