In order to be responsive, all bids must have the following with submittal: o Goods & Services Project Paperwork* o Line Items o Bid Guaranty (bid bond or certified check / cashier's check on a solvent bank) o Signed Contract by the Vendor* o Child Support Affidavit* o Bureau of Worker's Compensation Certificate o Insurance Certificate, naming the City of Toledo as an additional insurer o ANSI/NSF 60 Certification o Certifying Statement of lime samples quality shall be representative to vendors manufacturing or storage facility o Certificate of analysis for % Total Calcium Oxide, % total magnesium oxide, available calcium oxide and silica oxide o Submit slaking temperature as requested in item 5 of specifications o Submit pebbled lime sample to 3040 York Street Toledo, Ohio 43605 *Located on "Documents" tab in PlanetBids If using a check as the bid guaranty, the check must be scanned and uploaded with your bid.
