Refer all questions with regard to this RFQ in writing to mburroughs@smcps.org no later than Monday, February 28, 2022 at noon. St. Mary's County Public Schools (SMCPS) is requesting price quotes for the material, service, tools, delivery, and labor required to provide lead water testing and sample collection at school sites, as specified herein. o All testing procedures and processes will be in accordance with the Maryland Department of the Environment COMAR 26.16.07. or the U.S. Environmental Protection Agency, in the case where both agencies prescribe a procedure and process the more stringent will apply. o Vendor will provide the sampling containers, will collect the samples, test the samples and maintain chain of custody paperwork, and return the test results for each school site to the SMCPS, Department of Maintenance. o SMCPS will flush each school site 8-18 hours before the samples are taken and disconnect equipment as needed. o Vendor will provide a signed W9 with quote. o Vendor will complete the attached Business Relationship Affidavit and submit with their response. o Work will be scheduled with SMCPS, Department of Maintenance, and performed between the hours of 4:00 a.m. until 7:00 a.m., Monday through Fridays except when "all schools and offices closed" as indicated on the SMCPS operational calendar. The SMCPS' calendar and school closing updates are available for view online. o No vendor will be allowed to offer more than one price for each item or service even though the Vendor may feel that they have two or more choices that meet specifications. Vendors must determine for themselves which to offer. If the vendor should submit more than one price on any item or service, all prices for that item or service will be rejected. o Vendor shall ensure that all employees report to the main office in school buildings daily to sign-in and sign-out while work is being performed. o General terms and conditions are attached.
