Furnish all labor, materials, equipment, tools, transportation, and supplies required to complete the work in accordance with the Contract Documents. The intent of this contract is to provide a means whereby the DEPARTMENT can provide a quick response for implementing roadway improvements including, but not limited to: the installation of wrong way vehicle detection systems, install/repair/modify traffic signalization improvements, minor median modification, signing, pavement markings, minor concrete improvements, and pedestrian feature installations (crosswalks, ped buttons standard and accessible, mid-block crosswalks) for a period of two (2) years or until the contracted amount has been reached. Aforementioned works will be performed in Hillsborough, Pinellas, Pasco, Hernando, and Citrus County. The Contractor shall furnish all labor, materials, equipment, tools, transportation and supplies required to complete the work in accordance with the work plan, specifications, procedures and terms of the contract. The work under this Contract will be controlled by the issuance of Work Documents. All work in this Contract is considered specialty work. The number of work documents shall be limited by a budgetary cap of $2,000,000. The DEPARTMENT will not specify contractor crew size, work methods or equipment complements, provided the desired work quality is achieved as evaluated by the Project Engineer. The quantities shown on the Bid Price Proposal are approximate and only represent estimated planned requirements based on historical or specific project needs. The Department does not guarantee any maximum or minimum quantity, any range of quantities, or the exact quantities shown for each bid item. The Department's estimated quantities and the Contractor's bid price will be used to calculate a total bid amount. This total bid amount will then be used to determine the low bidder; however, actual payment under the awarded contract will be based on actual quantities completed. For this Contract in which specific sites are not identified at the time of letting, the Engineer will issue a Work Document. Work Documents will identify the location, description, amount of work to be accomplished, and the number of calendar days to complete the work. This is a district-wide push-button contract. The budgetary ceiling for this contract shall be Two Million dollars ($2,000,000.00) over the three (3) year term of the contract. Note the substituted Commercial General Liability Insurance requirements contained in section 7-13.2 and 7-13.4 of the project Specifications. Both insurance requirements are required for this proposal at time of execution. Prequalification Required. Contractors must have a current certificate of qualification in accordance with Florida Statute 337.14(1) and Rule Chapter 14-22, Florida Administrative Code, on the date of the letting to bid on construction projects over $250,000.00 as established by the Department's budget. For bids over $150,000.00, the standard proposal guaranty of 5% of the bid will be required, unless otherwise stipulated in the proposal advertisement. A Proposal Guaranty of not less than five percent (5%) of the total actual bid in the form of either a certified check, cashier's check, trust company treasurer's check, bank draft of any national or state bank, or a Surety Proposal Guaranty made payable to the Florida Department of Transportation must be received for each bid in excess of $150,000.00 A 100% Payment and Performance Bond will be required for all projects. All work is to be done in accordance with the Plans, special Provisions of the State of Florida Department of Transportation. FDOT - Call Order: 005 - District Wide Push Button Contract
