Contractor Awarded Address: Seattle, WA 98144-2051 USA Base and All Options Value (Total Contract Value): $175900.00 he purpose of this project is to demolish building 152 and perform site restoration. Demolition services shall remove footings, foundations, concrete pit, and utilities. The building was constructed in 1955 and has exceeded its lifespan. Pre-demolition asbestos materials survey and lead- TCLP testing was performed in 2018. All work shall be performed in accordance with federal, state, local laws and regulations, commercial standards, manufacturer's recommendations, and the Performance Work Statement (PWS). **This project is not fully funded at this time but a Short of Award has been issued. It is EXPECTED to be funded but not guaranteed. PURPOSE The purpose of this project is to demolish building 152 and perform site restoration. Demolition services shall remove footings, foundations, concrete pit, and utilities. The building was constructed in 1955 and has exceeded its lifespan. Pre-demolition asbestos materials survey and lead-TCLP testing was performed in 2018. All work shall be performed in accordance with federal, state, local laws and regulations, commercial standards, manufacturer's recommendations, and this Performance Work Statement (PWS). 1.2 SITE VISIT Interested bidders may attend a site visit prior to the bid due date. Interested offerors are encouraged to include any subject matter experts or sub-contractors required for this work. The site visit will be scheduled and advertised by the Contracting Officer (CO). The contractor shall verify all dimensions, elevations, and site conditions. INITIAL SUBMITTALS 1. Submit the following items no later than the Preconstruction Meeting a. AF 3064 (or equivalent) - Project Schedule b. AF 3065 - Schedule of Values 2. Submit the following items at least ten working days prior to onsite work a. Safety Plan b. Quality Control Plan c. Demolition Plan d. Abatement Plan e. Solid Waste Disposal Plan f. Dust/Debris Control g. Haul/Access Routes Plan h. Notification of Demolition and Renovation Form 3. The Government shall have up to five working days to review submittals B. PRODUCT SUBMITTALS 1. Submit DD Form 1354, Transfer and Acceptance of DoD Real Property 2. The Government shall have up to five working days to review submittals. C. HAZARDOUS MATERIALS 1. Hazardous materials must be approved prior to start of work. 2. Submit one copy of the manufacturer's Safety Data Sheets (SDS) for all hazardous materials used during the course of the project along with the following additional information: a. Number of components in material if it is a kit b. Container type c. Unit of packaging (oz, gal, lb, qt, ea, etc.) d. Package (can, drum, bottle, cylinder, etc.) 1.3 RESPONSE Submit cost proposals to the CO. All responses must be in writing, using the request for proposal (RFP) form specified in the solicitation. All responses must be received by the date and time specified in the solicitation. 1.4 CODES AND REGULATIONS A. All work shall be accomplished in accordance with the following regulatory codes: 1. AHRI Guideline K - Guideline for Containers for Recovered Non-Flammable Fluorocarbon Refrigerants 2. ASSP A10.6 - Safety & Health Program Requirements for Demolition Operations - American National Standard for Construction and Demolition 3. EM 385-1-1 - Safety and Health Requirements Manual 2 1.5 WORKMANSHIP All work shall be accomplished in a professional manner by experienced and qualified tradespersons to the complete satisfaction of the COR. 1.6 APPROVALS AND CHANGES A. Any changes to the terms and conditions of the contract will be issued in writing by the contracting officer prior to commencement of work or incurring costs. B. No deviation from approved plans, specifications and general notes will be permitted without written approval from the COR. Requests for deviations or substitutions must be submitted on a substitution request form available from the COR. C. Any contractor driven change request shall be submitted through a request for information (RFI). 1.7 WARRANTIES All work completed under this contract shall be guaranteed by the contractor against defects in materials or workmanship for a period of one year from date of final project acceptance. 1.8 SUBMITTALS All submittals listed below shall be submitted after award at least ten working days prior to use on site, or as noted below. The method of contractor selection has not been determined at this time. Estimate is at $200k - $500k *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
