BD 101656 Each bid proposal shall include all labor, material and services necessary to complete the projects in strict accordance with the project specifications as prepared and on file in the office of: ACM Engineering & Environmental Services, Inc. 839 Mill Lake Road, Suite 400 Fort Wayne, Indiana 46845 The Project Base Bid Proposal consists or the proper removal and disposal of Mercury containing flooring in accordance with all EPA, OSHA, state, and local re standards ana requirements in the Activity Room, east entrance area, and west entrance area at Shawnee Middle School. Wage rates shall not be less than the prevailing wage rates as determined pursuant to the Wage Rate Decision of the Secretary of US Department of Labor establishing the minimum wage rates which contractors shall pay skilled and unskilled labor. Bids received after that time or in another location shall be returned to the bidder unopened. Each bid proposal shall be accompanied by a certified check, a cashiers check or a bid bond executed by a surety company which is authorized to do business in the State of Indiana, made payable to the Fort Wayne Community Schools and in an amount equal to five percent (5%) of the bid proposal.
