This Request for Proposals (RFP) is for the supply, delivery, installation and maintenance of all labour, materials, equipment and tools necessary to provide environmental enhancements for various long-term care home locations of City of Toronto's Seniors Services and Long-Term Care Division. Environmental enhancements include but not limited to: Wall Murals Activity Stations Door Decals Snoezelen/Sensory Carts Activity Wall Panels Training for staff on proper use of the activity stations and snoezelen carts Scope of Work .1 Supply and installation of door decals. .2 Fire Retardant documentation for wall mural substrate and Murals Covering Resident Room Doors substrate. .3 Sensory activity/cart proposal. .4 Supply and set up of Sensory Carts as per proposal .5 Supply and installation of Wall Activity Panels. .6 Final walk through with client. .7 2-years - 4 quarterly review visits to inspect murals at no cost. .8 Repairs as necessary and based on Warranty document. The term of the agreement is to be for a period of one (1) year, with an option to extend the agreement on the same terms and conditions for an additional term of up to four (4) separate one (1) year periods.
